Welcome to Step 1 of the Serve Asia Journey! As you work through the application, take time to discuss this opportunity with your church and family. Be sure to spend time in prayer as you consider where God may be leading you.

Application Steps:

1. Contact the Serve Asia team for Application materials and instructions.

2. Read through the Application Checklist at the end of the Serve Asia Application Form. The following forms should be in your application packet:

• Application Form
• Pastoral Reference
• Local Church Endorsement
• Legal Waiver and Agreements
• Medical Questionnaire #1

3. Determine your type of placement and pay the Application Fee online or send in a check.

Please contact us to submit your application and to keep us updated as you begin the application process. Keep in mind that from the time the application is downloaded and started to the time of acceptance, the average person takes approximately one month.

A Note about the New Medical Clearance Requirements

As of February 1, 2008, there has been a change in the screening process of Serve Asia workers. This should make the process easier for those who need minimal medical screening.

Medical Questionnaire #1 - Please complete this form and return it to us as soon as possible. You may submit it through 1) fax, 2) scan/e-mail or 3) mail.

Medical Questionnaire #2 or other special forms - If we request this additional information from you, all information will be kept confidentially between you and our medical personnel. You may be asked to complete this second form to elaborate on questionnaire #1. Please uphold the integrity of our teams and long-term work by being honest and precise.

Medical clearance must be given by our medical personnel before you can be formally accepted into the program. We hope that this will be a simple and painless process!
